Update to Vendor Application Deadline

The deadline for Vendor Applications has been changed from September 15, 2015. Vendors who wish to be approved for OAS CAHPS beginning in January 2016 need to submit their completed Vendor Application by November 11, 2015 and need to attend Introduction to the OAS CAHPS Survey Webinar training session scheduled for October 28 and 29, 2015.
