Version 1.2 of the Protocols & Guidelines Manual has been released and can be found on the Survey Materials page. The following is a summary of the main differences between Version 1.1 and Version 1.2.

• Chapter III: Up to15 supplemental questions, and clarifications regarding Consent To Share Identifying Information question.
• Chapter IV: Additions to explain sampling more frequently than monthly, clarifications regarding Modifier 73 or 74, CPT codes for microsurgery added, clarification of information in files within Step 8.
• Chapter VIII: Clarification about providing response data to ASCs and HOPDs.
• Chapter IX: The code for Not Applicable is X, not 8.
• Chapter X. Clarification of the Vendor Authorization Tool, specifically the instructions about deleting a vendor.
• Chapter XI. Two explanations were added to the Patient Administrative records, one regarding coding of CPT and G codes into surgical category and one regarding lag time.
• Alternative Instructions for Scannable Forms correction of “If No, go to Q1.”
• CATI script English Q1 (“needed”), Q1, Qs10, 13, 16, 18, 20, 22 (upper casing of answer categories), Q12 (“side effects of the”), Q13 (“instructions”), Q15 (removed “can”), Q17 (“would you say”), Q22 (“any signs”), Q27 (age category for 45-54), Q31 (“Cuban, or”), Q32/32a/32b changes to race question as described in post-training Q&A document, Q34 (revised answer choices), Q35 (if other skip to Q35a),
• Sample Cover Letter 1st mailing Spanish: addition of final paragraph.
• CATI Script Spanish Q22 (removed skip), Q32/32a/32b changes to race question as described in post-training Q&A document, Q34 (revised answer choices), Q35 (if other skip to Q35a),
• Appendix K XML File Layout for Standard Header Record added
• Appendix L XML File Layout for DSRS Header Record added
• Appendix M XML File Layout for Zero Sampled added
