Vendor Authorization Form Now Available

Medicare-certified hospital outpatient departments (HOPDs) and ambulatory surgery centers (ASCs) participating in the voluntary national implementation phase of the Outpatient and Ambulatory Surgery CAHPS (OAS CAHPS) Survey in calendar year (CY) 2016 must select and contract with a CMS-approved OAS CAHPS survey vendor to collect and submit OAS CAHPS Survey data on their behalf. Before CMS-approved OAS CAHPS Survey vendors can submit OAS CAHPS Survey data to the Data Center, each registered HOPD and ASC must complete the online Authorize a Vendor form, which is now available on the OAS CAHPS website under the “For Facilities” menu.

Participating HOPDs and ASCs should note that The OAS CAHPS Data Center will not accept OAS CAHPS Survey data unless the facility has authorized the survey vendor to collect and submit OAS CAHPS Survey data on its behalf. If a survey vendor attempts to submit data and the facility did not authorize the vendor, the data will not be accepted.
