Online Exceptions Request Form Now Available

The online OAS CAHPS Exceptions Request Form (ERF) is now available to OAS CAHPS Survey vendors and can be found under the "For Vendors" tab once the
approved survey vendor has logged into the private side of the portal.

The Exceptions Request Form allows survey vendors to request a planned deviation from the standard OAS CAHPS Survey protocols. Examples of when to submit an Exceptions Request include, but are not limited to, requests to:

• Sample more frequently than monthly, including continuous sampling
• Exclude from the sample frame procedures which have eligible CPT codes but reflect procedures such as pre-admissions testing not appropriate for the OAS CAHPS Survey
• Use the disproportionate stratified random sampling (DSRS) method
• Mid-quarter data collection starts
• Include more than the allowed 15 supplemental questions in the OAS CAHPS questionnaire
