Minor Revision to Questions #5 and #6 in OAS CAHPS Web Survey Instrument

The phrase "at the facility" has been added to questions #5 and #6 in the OAS CAHPS web survey instrument, as shown below. The wording for these two questions now parallels the wording used in the mail and telephone survey instruments.
Q5. Were the clerks and receptionists at the facility as helpful as you thought they should be?

2 ▢ Yes, somewhat
3 ▢ No
Q6. Did the clerks and receptionists at the facility treat you with courtesy and respect?
