Updated OAS CAHPS Mail Survey Questionnaire and Telephone Scripts Now Available

The OAS CAHPS Survey Coordination Team has made some minor, non-substantive changes to the OAS CAHPS Survey mail questionnaire and telephone script for consistency across versions. All CMS-approved OAS CAHPS Survey vendors are asked to make these changes in their OAS CAHPS Survey materials at the next opportunity. The final mail survey questionnaire and telephone script in all approved languages are available here.

The changes made do not affect the content of the XML file layouts; therefore, no changes were made to the XML file layouts.

Changes to the OAS CAHPS Mail Survey Questionnaires

• Cover page: Added CAHPS trademark symbol (®) in the title of the survey. This change applies to the English, Simplified Chinese, and Traditional Chinese Mail questionnaires.
• Survey Instructions: Bolded “If No, go to #1” in the example skip instructions. This change applies to the English, Simplified Chinese, Traditional Chinese Mail questionnaires, and Alternative Instructions for Scannable Forms document in Simplified Chinese and Traditional Chinese.
• Survey Instructions: Removed the period after “If No, go to #1” in example skip instructions. This change applies to the English, Simplified Chinese, Traditional Chinese Mail questionnaires, and Alternative Instructions for Scannable Forms document in each survey language listed above.
• Survey Instructions: Changed first header of the questionnaire (page 1) “Survey Instructions” from English to the Chinese translation. This change applies to the Simplified Chinese and Traditional Chinese Mail questionnaires.
• Alternative Instructions for Scannable Forms document: Removed the underlining from the last sentence on the page of the Simplified Chinese Alternative Instructions for Scannable Forms document.
• Alternative Instructions for Scannable Forms document: Underlined the Survey Instructions characters in the Traditional Chinese Alternative Instructions for Scannable Forms document.

Changes to the OAS CAHPS Telephone Scripts

• Document Title: Added “(OAS CAHPS)” and the CAHPS trademark symbol (®) in the title of the survey. This change applies to the English and Spanish telephone script documents.
