OAS CAHPS Survey Online Discrepancy Notification Report (DNR) Form Now Available

The online Discrepancy Notification Report (DNR) form is now available on the OAS CAHPS Survey website. OAS CAHPS Survey vendors must complete and submit a DNR within 24 hours of discovering an unplanned deviation from the OAS CAHPS Survey protocols. Survey vendors can access the DNR via the "For Vendors" menu on the OAS CAHPS website once logged in. Instructions for completing each section of the form are provided in the DNR form.

More information about the DNR is provided in Chapter XV of the OAS CAHPS Survey Protocols and Guidelines Manual, Version 1.2.
