To obtain user credentials to access the private links on the OAS CAHPS website, each participating facility (hospital outpatient department [HOPD] or ambulatory surgery center [ASC]) must designate a staff member to serve as its OAS CAHPS Survey Administrator. To register as the OAS CAHPS Survey Administrator and establish login credentials for the OAS CAHPS website, you should complete the information below.
